The ACE assessment is more than just a quiz—it's a scientifically validated tool that reveals how early life stress impacts lifelong health. Developed in the original Kaiser–CDC ACE Study (1995–1997) with over 17,000 participants, it has transformed how we understand trauma, resilience, and healing.

What Is the ACE Assessment?

It's a short questionnaire (10 yes/no questions) that asks whether, before age 18, you experienced:

  • Physical, emotional, or sexual abuse
  • Emotional or physical neglect
  • Household issues like parental separation, substance use, mental illness, violence, or incarceration

Your ACE Score is the total count of "Yes" answers—ranging from 0 to 10.

Why It Matters: Validity & Reliability

  • First conducted with 17,337 Kaiser Permanente members in Southern California
  • Participants also underwent physical exams—allowing researchers to link ACEs with adult health outcomes in a robust, evidence-based way
  • Results have shown a dose-response trend: the more ACEs, the higher the risk for physical, mental, and social challenges

What Your Score Could Mean

  • 0 ACEs: Baseline risk
  • 1–3 ACEs: Moderate elevation in risk for issues like anxiety, chronic pain, or behavioral challenges
  • 4+ ACEs: Strong link to greatly increased risk—2 to 4 times higher—for chronic conditions such as heart disease, depression, smoking, substance use, COPD, liver disease, and early mortality. Also higher likelihood of adult relationship violence and incarceration
