Clinical Self-Check
Two widely used, validated screening tools — the PHQ-9 for depression and the GAD-7 for anxiety — offered here as a private way to check in with yourself.
This is for self-reflection, not diagnosis. These questionnaires are screening tools, not a clinical evaluation. A score here cannot diagnose depression or anxiety, and it is not a substitute for an assessment by a licensed mental health or medical provider.
Your answers stay private and on this page — nothing is recorded, stored, or sent anywhere unless you choose to email a copy to yourself at the end. Therapy Glow does not receive your responses.
If you are an existing client and would like me to have your results, you can forward the emailed copy to me yourself.
Optional. Used only to personalize the copy you email yourself — not collected.
If you're in crisis right now: if you're thinking about harming yourself or feel you may be in danger, please don't wait for this questionnaire. Call or text 988 (Suicide & Crisis Lifeline) or text HOME to 741741. If you're in immediate danger, call 911.

Your Results
What this means. These scores reflect how much you've been affected by symptoms of anxiety and depression over the past two weeks. Higher scores suggest more frequent symptoms — but they are a starting point for reflection, not a diagnosis. Many things can raise these scores temporarily.
If your scores are in the moderate range or higher, or if they've been persisting, it's worth talking with a licensed therapist or your doctor. If you'd like to explore working together, you're welcome to reach out.
This opens your own email app with your results filled in, addressed to you. Therapy Glow does not receive it. If you'd like me to see your results, forward the email to me yourself.
